Canada: + Procedure 1: Multiple- use Export Permit for Artificially Propagated Plants (4 pages) (These permits will be restricted to nurseries that have been registered under the Greenhouse Certification Program of the Canadian Department of Agriculture and Agri- Food (AAFC). The permits will be issued in whatever number of originals are needed in a year to export all shipments from a nursery.) + Procedure 2: Multiple- use Export Permit for Cultivated American Ginseng (1 page) (With respect to American ginseng grown in Canada, the same "Multiple-use" CITES Export Permit procedure will be used, except that the holder of the permit will be authorized to use photocopies of the permit to export ginseng. To be valid, the photocopy will have to bear a Canada Customs validation stamp.) + Procedure 3: Phytosanitary Certificate used for Certification of Artificial Propagation (2 pages) (For nurseries not participating in AAFC's Greenhouse Certification Program, the Canadian Phytosanitary Certificate used in conjunction with a CITES Inventory Attachment will be used as a CITES Certificate of Artificial Propagation.) + Procedure 4: Regular CITES Export Permit (This form will be used to export artificially propagated plants in cases where: A. a Phytosanitary Certificate is not required, such as for household plants to the United States of America; B. plants are not of Canadian origin; or C. plants are from Appendix-I species and are being exported by a nursery not participating in AAFC's Greenhouse Certification Program.) Denmark: + Phytosanitary Certificate (The front of this form is printed in green with the number in block 2. in red) + Phytosanitary Certificate for Re-export (The front of this form is printed in brown with the number in block 2. in red) (These forms replace those copies of which were sent with Notification to the Parties No. 770 of 31.8.1993) El Salvador: Export/Re-export/Import Permit (This form is printed on light blue security paper. Its actual size is 29.9 x 21.4 cm.) (This form replaces that a copy of which was attached to Notification to the Parties No. 670 of 16 January 1992.) Guatemala: Export/Re-export/Import Certificate (This form is printed on light blue security paper. Its actual size is 32.8 x 21.5 cm.) (This form replaces that a copy of which was attached to Notification to the Parties No. 720 of 21 December 1992.) Malta: Export/Re-export/Import/Other Permit (This form is the same as that a copy of which was attached to Notification to the Parties No. 734 of 1 March 1993. However, from 1 September 1996, it will bear an embossed seal instead of a rubber stamp.) Monaco: Export/Re-export/Import/Other Permit/Certificate Peru: Additional sheet (CONTINUACION PERMISO/CERTIFICADO No.) (A copy of this form was attached to Notification to the Parties No. 925 of 20 June 1996. Contrary to what was indicated in the remarks made at that time, it may also be used for trade in animal specimens.) Saudi Arabia: Permit to Import/Export Endangered Species (This form does not provide space for all the information required under Article VI of the Convention and Resolution Conf. 9.3. It should not be accepted by the other Parties if the required information is not provided on the form or on an attached sheet considered as being part of it.) Togo: Export/Re-export/Import/Other Permit/Certificate (This form is printed on green paper.) (This form replaces that a copy of which was attached to Notification to the Parties No. 701 of 24 August 1992.) Venezuela: Annex to Export/Re- export Permit (This form may be used as an Annex to the form a copy of which was attached to Notification to the Parties No. 670 of 16 January 1992.) 4.Finally, the Secretariat transmits to the Parties, for information, copies of the following documents: + the Beijing Statement on the Control of Wildlife Trade in the Asian Region; and + Protect Endangered Species, a leaflet prepared by the CITES Management Authority of Hong Kong.
